A: With either masking or surgery, leveling Connie’s maxillary arch should intrude her stepped down maxillary incisors—decreasing her excessive gingival display when smiling (Figure 3.94d). Of course, we need to examine her incisal display (with a relaxed lip) after her maxillary arch is leveled. In addition, post-treatment cosmetic dentistry (maxillary incisor composite or porcelain veneers) could improve her smile by addressing her worn incisors. Otherwise, correcting her anteroposterior relationship with masking would not help her smile, unless her face height increased with mandibular posterior tooth eruption (during leveling of the COS). If her maxillary incisors were erupted with masking (heavy Class II elastic wear), then her incisal display when smiling could increase. A mandibular advancement surgical procedure (with genioplasty setback) could increase her vertical dimension (improve her facial balance) if the mandible was moved downward as well as forward—but it may not improve her smile. A final, specific, treatment plan to address her smile esthetics cannot really be made until after her maxillary arch is leveled and aligned with fixed orthodontic appliances (reduce unknowns).
